Please download and complete all required forms, and either mail or email them to us. The Center requires these forms to be completed before the initial visit so that we may have time to review them before evaluating your child.

We are aware that in your search of answers, you have no doubt been faced with many forms that you needed to complete in order to see the doctor. If you have already filled out a Questionnaire for Children with Autism & Related Developmental and/or Attention Problems Form, you may simply bring this with you to your appointment, along with any medical history reports.

If, however, you have never filled out this form, it is important that we have it on file. You can download the form by clicking the link below (due to the size of the document, it may take a moment to appear) and fill it out on your own time, so that you can immediately visit with the doctor at your appointment.
